To renew CNA Certification in Delaware:
(1) A pre-printed recertification form will be mailed out to the CNA 2-3 months prior to your expiration date. If you still do not receive any notice, you may obtain renewal form by contacting Delaware Nursing Assistant Registry or visit D&S Diversified Technologies.
(2) Requirements to be met:

You must perform at least 64 hours of nursing care services for the last 24-month certification period.

If your address or name has changed, please call the DE CNA Registry to report the change. If your name has changed, you must submit a copy of a legal document showing your name change.
(3) Completed form with required documents must be mailed to the address indicated in the form.
(4) If you have not worked atleast 64 hours of nursing care services for the last 24-month certification period, you must re-take the State examination.
To renew Expired CNA Certification in Delaware:
(1) Contact Delaware Nursing Assistant Registry or visit D&S Diversified Technologies.
(2) If the registry provide form, complete the form with required documents and fees. Mail the form to the address indicated in the form.
